Insurance:
I can assist people with accessing their out of network benefits by supplying monthly invoices upon request for services rendered. I am in network for the following insurance plans: - Optum - United Healthcare - Cigna - Evernorth - Aetna Please note: For the insurance types that I am currently not in network with, clients can be provided a monthly invoice statement to submit for out of network benefit purposes. The out of network benefit process is up to the client to discuss, assess availability and expedite with their individual insurance carrier. |
Hourly Rate
$150.00 per 55 minute session. Payment is expected at time of session for out of pocket payment clients, deductibles related to insurance, and those accessing their out of network benefits. A 3.5% fee will be added to all credit card transactions. Office Hours: Monday 10:00am to 7:00pm Tuesday 9:00am to 5:00pm Wednesday 10:00am to 7:00pm Thursday 9:00am to 1:00pm Friday - Closed Saturday - Closed Sunday - Closed Cancellation Policy: Failure to show or failure to reschedule your appointment within 24 business hours will result in a $75.00 no show fee. |
